Chapter Page Update Form

Use to update any outdated or incorrect information on your chapter page on the Washington State Web site. Choose the category below that needs updating, then click the "Submit" button below.
The form results will be sent to the Washington State Directors and Webmaster for processing.

Meeting Day and Time:
(Type into field the usual week of each month you meet and the time, i.e. 8 a.m. breakfast, 9 a.m. meeting.)
Meeting Location:
(If your chapter has a fixed location, type its name, address, city, state and zip. If it changes, type "Call or email CD for Info.")
Directions to Location:
(Give brief instructions on how to get to your meeting location if it doesn't regularly change.)
Annual Event:
(Brief description, and location of your annual chapter event that is open to all who wish to attend.)

To add CD and ACD pictures to your chapter page, attach them to an email to the Webmaster. Include names of persons in each picture and position held in the email.
